The AGD has tried to make recordkeeping easy by giving you a variety of tools/templates you can use in your programs. All examples can be found in the PACE Provider Tools section of the AGD Web site.
You can use this roster as a sign-in sheet at all of your courses. Download it to your computer and modify it as needed to accommodate any additional information. Make sure you don’t remove any of the fields; all of the information requested on the verification form is needed by one or more licensing boards if the hours are to be accepted for license renewal credit.
Every person who attends your program must receive some form of verification that they can maintain in their files as proof of attendance. You can either distribute this verification at the end of the program or mail/e-mail it to attendees after the course (if e-mailing, make sure the information cannot be changed easily). For attendees to receive credit for the program, the verification must contain specific information. This information varies from state-to-state and province-to-province. To make it easy for you, the AGD has researched all North American licensing boards and consolidated the information into one sample form. Since you won’t know all the areas where your attendees are licensed, PACE expects you to include all needed information on your verification. This includes:
· Organization name

· Participant name
· AGD member ID number – If available
· License number – Providers can leave this space blank so participant can fill the number in themselves.
· Course date(s)
· Course location – City and state are fine
· Course title
· Course subject code – Not sure? Share the AGD subject code list with your instructor(s) before the program and have them identify the code(s). Hours can be divided between two or more codes if appropriate.
· Educational methods – Designate the hours as Lecture, Participation, Self-Instruction, or Teaching (for instructors only).
· Number of CE/credit hours – Classroom hours are awarded on an hour-for-hour basis. Self-instruction, Protocol/Mastertrack, or Teaching hours may be calculated differently.
· Course completion/verification code – Providers randomly assign and record verification codes.

The AGD requires PACE-approved providers to submit verification of AGD member attendance to the AGD within 30 days of course completion. The easiest way to do this is use a sign-in roster that allows all AGD members in your course to identify themselves. Make it a habit to enter the information right away online at the end of the course, or fax (to 312.335.3432) or e-mail the completed roster (to membership@agd.org) immediately to the AGD so that our membership team can add the information to our members’ records quickly. The AGD strongly recommends using the online roster submission form, the only method that will give you a quick reference as to what you submitted. Since you will only submit CE information to the AGD for AGD members, other participants might ask about what they perceive as “preferential treatment.” Use this quick script at the beginning of your course to explain the process to all your attendees. You can also use these talking points to answer additional questions.
