Rules & Eligibility1. The 2009–2010 Refer a Colleague program runs May 1, 2009, through Apr. 30, 2010. Applications and dues must
be received and processed on or by Apr. 30, 2010, to qualify.* Applications and payments processed after Apr.
30, 2010, will be applied toward the 2010–2011 Refer a Colleague program.
2. To be recognized for recruiting a member, your name and AGD ID must appear on your referral e-mail sent to
marketingdept@agd.org, your new member’s online or hardcopy application, or on either of the AGD referral forms
(online or PDF version); a new member is officially credited to the recruiter when he/she is accepted for
membership and all dues have been received.
3. In order to be considered for the monthly raffles or the Top Recruiter prize in either the Grassroots or Volunteer
Leader recruiter categories, your new member(s) must be an active general dentist member. Associate, affiliate,
and dental student members are not included in these prize categories. An active general dentist member is
defined as one of the following:
 A dentist who graduated from an accredited dental school in the U.S. or Canada; A dentist who has successfully completed an accredited general practice residency (GPR) or advanced education in general dentistry (AEGD) program in the U.S. or Canada; or
A dentist who holds a license to practice dentistry in any state or territory in the U.S. or province of Canada, but is currently practicing in a country outside the U.S. or Canada.
4. In order for your constituent to be considered for the Top Constituent Recruiter of Students prize, your new
member(s) must be a dental student member. Active general dentist, associate, and affiliate members are not
included in this prize category. A dental student member is defined as a pre-doctoral student of an accredited
dental school in the U.S. or Canada.
5. For each new active general dentist member you recruit each month, your name will be entered into the monthly
recruitment raffle; the more new members you recruit each month, the greater your chance of winning.
6. Program rules and prizes are subject to change by the AGD; any changes made to the program will be posted to
this Web page.
* Mailed applications and payments may take up to two weeks to fully process; please keep this delay in mind when
sending in applications toward the end of the program. For fastest processing, the AGD recommends faxing your
applications and credit card payments to 312.335.3443.
