General Information

You can submit CE credits to the AGD in three ways:

  1. Fax (For fastest processing, fax your CE submissions):
    1. CE credits: 312.335.3432
    2. Credits for teaching, published articles or textbooks, formal post-graduate, or residency programs: 312.335.3431 (Written verification of the activity must be included.)
    3. Self-Instruction Program: 312.440.4261. If you are an AGD member and subscribe to this program, the CE credits you earn will be automatically added to your transcript as self-instruction.

CE Submission Do’s and Don’ts:

  • DO make sure you follow all of the requirements on the CE submission checklist.
  • DO submit CE course information directly to the AGD if you’re an AGD PACE-approved provider. Please allow up to 60 days for credits to post from AGD PACE Providers
  • DO provide your six-digit AGD membership identification number when attending courses with a PACE-approved provider.
  • DON’T use highlighters and markers to highlight course information forms.
  • DON’T submit CE course invitations, registration confirmation slips, receipts, or cancelled checks as course verification. They cannot be processed as records of CE.
  • DON’T use abbreviations for program provider names or course titles.
  • Always keep a copy of your course attendance verification for your records.
